It's pretty basic.  An invoice is a piece of paper with the following information on it:

Company name (Or yours)
Contact info
Who it is to (who ordered whatever)
Price with breakdown
Date
Due date
Tax #s, depending on country
Invoice #
Maybe there is more, depending on the local laws, so probably just look at any invoice you got to see what they included.....

Payee:  LTRsubNW, 2% 10, N/30.
