i am very seriously considering a frenulum piercing where my Mistress could apply a leash.  i am very concerned about the pain (i am a devout coward) and have heard from some that penile piercings heal poorly.  As i am also a diabetic any risk of infection is very worrisome to me especially there.  does anyone here have much experience with this specific piercing that they could share with me?

Genital piercings heal great. The skin where I think you intend on placing a piercing is prone to tearing and should not be pierced.
Find someone with great piercings and ask them who did ther piercing

Are you cut or uncut?
On the plus side, genital piercings heal quickly because they get a lot of blood flow. However, the other rule of piercing is that the more parallel the piercing is to the surface of the skin, the more likely it will be to grow out. For example, earlobes, nipples- the needle goes through perpendicular at both sides (usually), so they generally heal well. Surface piercings, like the nape of the neck, sternum, etc., almost always grow out. If you're cut and considering a typical "frenum" piercing, meaning a straight bar across the underside of the penis below the glans, you're generally doing a piercing on a curved surface. As such, when you're hard, the bar will remain straight while your skin tries to stretch away from it, and you'll most likely end up "losing" some of the depth of the piercing over time.
Also, because this skin is relatively thin and stretchy, it's not the best choice for a leash. A "Prince Albert" works better, because it's anchored in the stronger flesh of the urethra. If you do get it done, get it done with the thickest (smallest numerical gauge) your piercer will use, for greater strength. The pain is pretty minimal.

My former slave had his frenum pierced as his mark of ownership from me.  He was deathly afraid of needles and chose this route because of the sacrifice.  While he almost passed out during the piercing, it was over in seconds and we enjoyed quite a bit of pleasure with his "new toy".  He was meticulous with the healing process (about 2 weeks) with soaking in sea salt, cleaning with provon soap, etc.  He had no health comorbidities to be concerned with.

Speak with a master piercist or your health care provider concerning any "incompatibilities" with your DM.  I don't know how stable a diabetic you are, and that would be of concern if you're brittle or have had septic issues in the past.

My very first piercing was a frenum piercing...1/2", 14g barbell.  Surprisingly little pain, healed very quickly...no problems other than it grew out after about nine months.  Got it repierced, grew out...followed this cycle a couple more times.  It, along with all the other piercings in my penis, are now gone.  They were causing problems during intercourse.
 
I'm cut...so it was not an issue for me that way...don't know if it can be as easily done (or done at all) if one is uncut.

If you want a leash go with the PA (prince Albert) I have played with many of them, and know countless people with them, and they all say the same thing, it felt like a little pinch and a slight burn afterwrds.

i must admit the very thought of a piercing through the urethra scares the willies out of me!  i have heard from several sources that these are more stable long term than frenulum piercings.  my question in that case would be the effect on the urinary stream, do you just spray all over the palce or is the stream stll controlled despite the object in the urethra.  Also how is the healing of this piercing since it involves the urethra and more depth of tissue.

PA's take longer to heal.  Depending on how the piercing is placed, and how large they stretch it, some guys do find the need to sit when they pee.

An alternative for leash placement would be a guiche. The guiche is placed in the "taint", basically the nerve reaction spot between the scrotal sac and the anus, in the perenium.  (Watching them place it is an amusing activity.) I've seen some pretty tough guiche placements, and they've not been too fragile not to hang weights and chains from.
