I've been taking HRT for five years now, and androcur (a testosterone suppressant) for three years. To have any effect with HRT alone, you would need to take 2-3 tablets a day of the most common forms of female hormones - usually the contraceptive pill. Penile function can be inhibited by this regimen, but by no means is this common; erection and ejaculation remain possible in many cases. I also tried the cream for a year or so, but with little or nothing by way of noticeable results. It is supplied in the UK at least, in a pump which produces the cream in measured doses. The real difference came only when combining androcur with the HRT; testosterone was inhibited, allowed the HRT to work pretty much alone. By way of the androcur however, penile function is very much inhibited - erection is very occasionally still possible, but ejaculation ceases altogether. Also, sex drive drops, presumably because of the absence of testosterone in the system, or changes in terms of arousal cues and expression. The safest way to proceed is by way of your doctor, who can prescribe these hormones subject to need. Most usually, upon a diagnosis of gender dysphoria and a decision to treat by way of gender transition; the process for such treatment is governed by an initial and ongoing psychological and psychiatric assessment. Although they may also be prescribed for other situations - for instance androcur is used as a treatment for prostate cancer. Androcur is also used for the chemical castration of sex offenders. It is also possible to obtain HRT and androcur through the black market. This is inadvisable however, since by the nature of it, it is not done under the observation of a doctor, and you have no idea what it is you have. Also available on the black market are some very strong treatments which are not used by doctors on medical grounds. HOWEVER No one should "play" with hormones; changes will not be simply the growth of functional breast tissue, but also general in nature, most noticeably in terms of the redistribution of body fat to a more female pattern, general laying down of a layer of fat all over the body as in the female form and loss of musculature. In addition there may be weight gain. Changes may be irreversible. Also, there may be adverse psychological effects from such a treatment. These effects are used by some practitioners to discern between fetishistic crossdressers and similar who like the idea of becoming female, and transsexuals. The loss of penile function for the former is disastrous and usually results in voluntary withdrawal from treatment and is often accompanied by depression. The most important possible adverse effect however is the potential hepato-toxicity of such a regimen - liver damage can result. For this reason, regular blood tests for liver function are necessary.
