Anal fisting is just anal fingering taken a little further, so I imagine a number of people do it without whips and chains. I don't spend a lot of time discussing sexual proclivities with people who don't do BDSM, so I don't have a personal story about thus and such otherwise-vanilla person who likes getting hands put in his or her ass. But there are handballing enthusiasts (often men who like to play with men) and guides written about it (Trust, by Bert Herman) that don't treat it as merely a subset of BDSM. It's a possible adjunct to BDSM, like penis-vagina intercourse, oral sex, and vaginal fisting, but like all those activities, you can do anal fisting without pain, negotiated inequal-power, or bondage.
Edit: Vaginal fisting is definitely independent of BDSM! I've done vaginal fisting without any pain, bondage, or proper d/s, and I fisted a woman before I knew that there were four letters to describe my "rough sex" proclivities. I also have known women who enjoy receiving or giving vaginal fisting and don't identify with BDSM at all.
