I've been trying to find a photo/video release form online but haven't had much luck yet. Does anyone know of a good site for a photo or video release form? Or does anyone have a release form I could adapt to suit for my purpose?

Why don't you just go to the local mall and ask the photo shop there for one? I'm sure they would be more than willing.
