Fecal incontinence is the least of your concerns. That's related to the sphincter muscle and stretching it too fast, too wide or for too long at a time, from the handballing sites I've read. And in most cases, it seems you can deal with it using a buttplug, which would be more appropriate than diapers or depends for those of us in the community, I should think. Surgery can also apparently correct some issues of this sort. Talk to your doctor about it, though.
At a certain depth (you should be safe up to about 15cm or so), you find the start of the sigmoid colon. The rectum itself is surrounded by muscle, and thus not quite as sensitive as the rest. However, the sigmoid colon bends (hence the name), and it is about as strong as a wet paper towel. This can make it complicated to move past it, and should probably only be done by someone with a lot of experience or significant anatomy skills. Perforating it is Bad(tm): very painful, and needs immediate intensive care.
Once you've moved past the sigmoid, you're moving to the side and upwards, past some very sensitive organs and a couple of nerve trunks. Then there's a bend below the diaphragm, a fairly straight path to the other side of the body, and then another bend before another fairly straight path back down. I remember reading someone saying that the nerve trunks can be stimulated in a fatal way if you're not careful.
There are people going as far in as their shoulder (bending their arm and effectively stretching the intestines a lot), but I certainly wouldn't recommend it to anyone who isn't an expert at these things.
Personally, I'd probably stick to the rectum, but your definition of "acceptable risk" may be different from mine. I'd talk to a doctor about it, though.
If he doesn't believe you, offer to have your mistress come along to demonstrate. He'll probably decline, but he'll also probably realize you're serious. If he doesn't decline, it's a perfect opportunity for some humiliation and getting your hands on medical-grade lube (exploration cream, I think they call it). [:D]
