Cystic acne is acne that has pitted and scarred the skin badly, and in many cases, develops semi-permanent hard lumps on your skin. They can be colorless or red, but you'll know if you have them. It's pretty much a hard scar with fluid in it. The only way to treat them is to go to a dermatologist and/or usually have laser treatment done.
