The military taught us as everyone else has already answered.  It goes over the top and to be correct you fold the edges over to make a little triangle for you to have something easy to grab ahold of.

Wow people actually put the toilet paper on the roll?? I thought my ex wife just bitched about that to complain...huh. I like to set mine going to the right...while its standing on end on the back of the toilet above whatever book I may be reading at the time.

I put mine under instead of over the top because it leads to waste to put it over. I am not a wasteful person.

And yes, I believe it goes overtop.  While we (women) shouldn't be using TP with scents or colorful designs...they ARE made to be shown off when the paper has been installed properly.
 
Regarding cats in the bathroom?  Allowing this can cause serious water wastage!!!

gotta be over the top....i hate fumbling for the end in the dark!

as far as the cats....they are nice enough to leave the roll on the holder alone....it's the others UNDER the sink that fall prey to them!  They figure if it ain't up there, it's fair game....goes for the paper towels too!

lol..  it is always, always over the top.  
And yes I also have issues with this.  I change it at work, in a public bathroom, at other people's houses, etc.

the maid does over the top with a pretty Merry Maids gold embossed sticker

and you've only mentioned (really) cats

if you have little kids?  its best kept in another room! under lock and key preferably - like the "Pay Before You Pump" - at the gas stations you sick people!!!!!!!

we have a little boy who has  flooded the bathroom because of too much toilet paper - he flooded the bathroom 2  inches deep - down the heater and into the basement - thank god the hardwood hadn't been installed yet! lol

now that I know how to use the three seashells, I have no need for your primitive handfuls of wadded paper.
