People who are condescending is a big one for me. If you really are intelligent and experienced you wouldn't need to advertise that fact on a continual basis. That and people who think that because person A is a Dom and person B is a sub and they live within 6 hours of each other, that they MUST be a match as a couple.

Oh.....and guys with combovers....seriously, just let it go. Bald can be very sexy and you're not fooling anyone.

Physical....  I think W/we can go ahead and accept that bad personal hygiene is a given.  Unless that's your personal kink, then go for it.  Seriously obese (not saying it to offend, but it's a preference) is on the list.  Hair for Me is a requirement, but the come over thing is absolutely a turn off.  Bald won't work for Me, even if you try hiding it. 
 
Mentally.... I suppose you can lump closed minded and unwilling into this catagory.  Add Me in for the catagory littlesarbornn nailed so perfectly as far as how I see the lack of intelligence (specifically, not that they don't know, but they aren't willing to learn).

smoking is a definite no-no
excessive body hair (well, body hair at all) but that can be taken care of ...
arrogance
stupidity/willfull ignorance
a "you aren't Dominant enough to Dom me" attitude
inability to follow directions
unwillingness to at least try and type out legibly (I'm not a grammar or spelling nazi, but my dyslexia makes it difficult enough for me without trying to translate horrible grammar/spelling and/or netspeak)
poor hygiene
bad manners

i would say, people who leave their mouths hanging open, an excessive interest in pop culture (celebrity drama, reality TV), lack of intellect (in my case, someone that you can explain things to repeatedly, and they still won't "get it"), arrogance, lack of interest in tying me up and beating the crap out of me [&:]

Physically, significant body/facial hair, overly skinny (lots of ribs, pointy hipbones, etc)

With me it happens to be women.
 
Physical; a) Bad teeth.
               b) I have dealt with having a partner that had beautiful teeth
                   but a medical condition  that caused bad breath, really tuff
                  situation for me.
               c) Lack of self care.
               d) Extreme Butch.
               e) a lot taller than me, I like a partner who is close to my height or bit shorter.
 
Mental;  a) Lairs.
             b) Has no interest in animals.
             c) Is mean, nasty for no reason, avenging.
             d) Crazy in a scary way, I like crazy in a good way[:D].
             e) Stone cold ugly.
             f) Lazy. 
             g) Dosen't get or doesen't connect naturally to the humour that abounds
                 everyday.

My biggest turn off is someone who constantly wants to tell me what they want. As in I want you to spank (gives details as to how it is to be done)  I guess topping from the bottom is the best way to say it.

Generally speaking, SAMS.  Poor hygeine (can't be said enough).  Lack of motivation.. Excessively overweight.  Couch potatoe... Topping from the bottom.  A do-me list.  No teeth.  No job.  Overindulgence in alcohol.

In many ways the opposite of your own turn offs... I dislike skinny/overlyfit bodies in general. 

Anyone with the IQ lower than the average 3rd grader. I like for my men to know and put to use the difference between your and you're! I'm not expecting for someone to know how to spell metacognition here, or even know what it is... Just the basics! :)

Physically, it would have to be when a woman does not care about her appearance.  Not, that I have to have a supermodel or an athlete but it is important to me that she care about her looks. 

Mentally, it would have to be a woman who can't keep up with me.  Again, she doesn't have to be a Rhodes Scholar but she would be able to carry on conversations on a variety of subjects and also be willing to put up with my sense of humor.

That is the worst thing I have heard in a long time!
