I dispise questionares also .. Id rather have the direct answers Myself that way no if and or buts can be in there ..My opinion...

Here's the one I use for all my slaves. Do you like me? (Yes) (No) If you answered, "no", please write a 22,000 word document explaining why, or you may change your answer in the previous question. For some reason, the babes just aren't flocking to my nest.

without reading 5 pages... i once filled out a sheet like that for a Dom... if i had a copy, i wouldn't hesitate to send it along. alas, i dont anyway... it was very detailed... asked me if i liked something... then how much (a number 1-5) and then if i hadn't tried it yet, did i want to.. all on excel with drop down menus... and so many different things that i had to look half of them up to know what they were. it was a great tool for me as a submissive to learn about myself and provide that information for my Dom.
