We seem to get a fair few fantasists in here asking questions about this stuff from time to time. Often theyre immediately identifiable as such, and just as often I "give 'em the talk". Whilst I'm not sure about the OP, the enquiry seemed to lack those elements of the fantasist, so I decided not to "give the talk". Its really, (I mean really) not a good idea to do this stuff if its not something you need to do. But then its really not that easy to do it at all without the psych approval - no surgeon in their right mind is going to open themselves up to the potential problems of operating without psych approval. The only issue then remains whether its a genuine need and the person can convey this to get the psych approval - albeit that its regrettably, suicidally possible to get the psych approval if one is convincing enough, regardless of need, and there's plenty of coaching material available.
