My favorite: a medical room, equipped with screens/curtains, metal hospital bed etc Medical waste disposal bins would be thoughtful, although members will probably have their own for their own sharps.
I love rolling stainless steel morgue/autopsy gurneys for bondage, CBT, necrophilia play etc, but I know they are not easy to acquire.
Old fashioned dentist chairs are popular too in medical rooms, but dentistry is not my kink at all (eeeeeek!!!).
Dungeon basics: a few really sturdy St Andrew's crosses (to take heavy weight plus force - including fainting 300 lb men - without collapsing), spanking bench(es), human sized cages and a block and tackle securely attached to the ceiling able to suspend 3 or more people in bondage and/or any of your cages with people locked in them
Latex disposable gloves and pump packs of lube would be really handy
Bowls of plastic pegs (clothespins). Always popular.
Spray disinfectant, spray disinfectant, spray disinfectant and anti bacterial handwash too. Some of us are obsessive compulsive germ freaks and want to spray before we play.
Enough bathrooms!! It ain't easy getting in and out of rubber to go to the loo. Extra talcum (baby) powder in the bathrooms would be thoughful.
Optional but great: a very sturdy cube for shibari , web bondage, suspension bondage and a sling, preferably with a mirror on the backing wall (so you can work from behind and watch the front through the mirror). Construction scaffolding painted black works well at a pinch if the club cannot afford heavy timber just yet
A few slings for fisting etc (attachable to the cube or the ceiling)
DVD player and data projector running BDSM movies on blank walls (tasteful ones, if you can)
A Mistress chair (throne) in each dungeon, preferably with foot stool (coz working in thigh high stiletto boots hurts like hell after a few hours!)
Bins to throw the house's toys for cleaning straight after use, if the house plans to provide pegs, rope and other non-insertibles (nothing worse than picking up sticky things by accident!!)
Some sensible rules about bringing your own insertibles, not interrupting scenes, approaching the dungeon monitors with any concerns, chatting in the chill out area not the dungeons, some dress code (vexed issue I know, but being watched by strangers in street clothes can throw out your head space mid-scene), no photos or video without permission of the house, right to remove anyone drunk or suspected to be on drugs etc
Good security including someone willing to walk women to their cars at the end of the night
Dimmer switches on the lights that work. Bright lights and loud music really kill the atmosphere.
If there is sex on premises, closeable room(s) with beds and free condoms to encourage the swingers/exhibitionists to do it in private. Watching other people hump is not everyone's cup of tea and having them hump next to you while you are trying to work is distracting (in my prudish opinion  ).
Similarly, rules banning masturbation while watching other people play unless they give you permission (its totally gross getting wanked on by a stranger, but that has only ever happened to me in Berlin, never the US, UK or Australia, so it might not be a real issue).
Secure cloak room for your street clothes, toy bag, handbag and car keys etc. Its no fun schlepping your stuff dungeon to dungeon all night hoping it doesn't get pinched while you are busy
Controversial but it works: Mistresses (frequently paid) to do demos, play "hostess" to newcomers and introduce them to others (especially other first timers). Visitors - especially those coming alone - who feel like they made 'friends' on arrival and had something to watch are more likely to return. Anyone can meet and greet of course, but a straight/ pansexual club with no Mistresses - or only Mistresses that are all already taken - doesn't seem to stay in business for long (IME only - of course others might have a different experience)
Eeer.....I want rather a lot compared to everyone else. Sorry!!! Please let us know when you hold your Grand Opening.
