My suggestion would be either very coarse sandpaper or steel wool either of which is taped inside the knee pads.

Ahh, darn you, stella! I was gonna say sandpaper too, hehe. Great minds think alike.

Or a less-messy version of the snow idea: tie plastic ice packs to his knees. They would certainly be uncomfortable... due not only to the cold sensation, but also the raised lettering on them. There would be some water due to condensation, but I'm sure he'd be eager to get on his feet to mop it up! Or you could use the bag kind (here) and put it in a ziploc baggie with beads/rice/what have you.

Get a humbler... nothing to do with the knees, but it does make crawling fun.

Oh what the hey, why not go ahead and amputate at the knee.

Require them to keep the wounds raw with steel wool.

Sanitized of course, safe, sane and all that you know.

Then you can embed sockets for the easy snap on of wheels.

Ya know, just for a little added humiliation as they zip down
to the local DollarStore for some feminine napkins on those
heavy flow days when you're a tad bit bitchy.

Aw, this is evil, Coupleofwhats.  Men's knees are knobbly (why they're differently shaped to women's beats me.  What evolutionary advantage would knobbly knees offer?  Question for a different thread.).  Mine are even knobblier than average, I think.

I'm with Snow: it always was painful to crawl around for long, no matter what. age.  I'd say: check the shape of his knees first before giving him too much torture . . .

Those little plastic discs that go under furniture that sits on carpet are awesome, too!  They can be placed inside a loose fitting shoe, or in a back pocket, etc.
