One, in my experience the most comfortable position to sleep with your hands behind your back is sort of half-curled up in a semi-fetal position, and twisted somewhat forward so that your weight is on the front of the shoulder that's in contact with the floor. This is much easier on the shoulder than lying directly on your side, and makes it much less likely that you'll cut off the circulation in your arm. Second, yoga is invaluable. Regular stretching exercises for the shoulders and elbows (which are often overlooked) will do wonders for keeping those joints loose and limber during the long nights. Oh - and third, it's helpful if you can learn to half-wake up frequently throughout the night and switch sides. Hm. OK, that was three suggestions instead of two. Sorry about that. Just pick whichever two sound best, and ignore the other one!
