I watch Twilight and i get hot and bothered every time edward looks sultry and smoldering, I pretty much cum in my pants when he growls at the would be rapists, its not real and to think it is real would be a little mental but it turns me on, it really doesn't stop it being hot, i fantasise about being bitten by a vampire for a million different reasons but it doesn't mean I want someone to attack me and drain all my blood (I would like a safe vamp scene but thats something else)
There has been times that I have sat down and imagined shooting someone in the head, doesn't mean I would ever do it, our mind is pretty wonderful and if we wrote down every slightly distasteful thought we all had then we would have massive volumes sitting on our shelves.
Fantasies are often the safe venue we have to think about things that we would never actually want to do in real life, often extensions of things that we we actually do.
As to what you think is extreme, you have to remember that not everyone has the same ideas, also a lot of our every day lives are unsafe so I always think its about minimizing risk rather than avoiding being unsafe or else I would never leave the house, and that would be sad, there be people out there
