First, let me express how VERY sorry I am that the two of you are going through this very difficult time.

Second, to answer your question, you need merely rephrase it from "When your Master is sick" to "When your PARTNER is sick" -- and THERE'S your answer. Take the BDSM out of it.

Surely care can be dealt with as a control element and can in fact help to enforce the relationship. for those times when his health is affecting him to the point he either cannot communicate or is just so unwell that 'lifestyle' needs to take a back seat; simply have advance instructions in place for this. this means that whatever you do for his care, he will have instructed you in this prior to the situation. this means you should still feel comfortable with regards your submission and still provide whatever is needed that currently feels outside of instruction.

OP..

My impression is that you seem to be doing a pretty awesome job already, and I really wish you and your Master well.

When someone is ill, it really is hard to strike the balance - and you'll have to feel your way towards that balance (and if you're human you'll get it wrong from time to time).
