The people I know who have eternity collars have large enough ones that the collar rests on the collarbone. It gives roughly enough room for someone to grab the collar in a fist, depending on the size of the hand. If the person has a 13" neck, a 14.5"/Small collar should fit just fine with plenty of room for comfort. You can get it smaller or larger, depending on how you want it to look. Eternity collars are designed to be worn 24/7, and the people I know who have them only take them off for medical proceedures. They lock with a screw that takes an allen wrench, so they're almost seamless. Most people don't wear pendants because they want the collar to look like what it is, but that's a personal choice. I've seen some people wear slide pendants on them for special occasions whey they want to look more dressy. I would suggest caution if you decide to get a pendant because it has to be able to stand up to sweat, showering, etc. and also be comfortable enough to sleep in, as you pointed out. How comfortable the pendant is will depend on the type you choose. The collar itself seems to be very comfortable. You could always get one of the colored ones and skip the pendant. The rainbow one looks especially like jewelry, IMO.
