Colon condition is going to be important, as is mental attitude - that you have pain with large BM's indicates dehydration to me, hydrate more, and sugar (ingested) will draw more water into the colon. You can get used to anything, but yes, anal sex is in some ways, a lot like taking a shit, the same autonomic reflexes are involved to some extent, and medically speaking, it isn't, or shouldn't be, any more harmful than taking a big shit - if you're the catcher her, I hope you've chosen your pitcher carefully though, there will be some discomfort probably, but in your case, lube and lots of it, is definitely called for, and some stroking to get you in the right space mentally would also be a good idea.
