And the explanation you asked for has nothing to do with the anatomy of a dog. It has to do with Sick man trying to deal with his illness by saying, "IT ain't me, It's YOU!" That you can hear him say it, from the point of view of bearing your life, is merely an accident. You happen to be standing there when he says it, so you hear it.
