Mostly by family members finding loved ones in none to flattering condition post mortem. As for some of the risky practices we do considered manslaughter if it results in death, yep. If something a person does directly or indirectly causes a person's death, it it at the minimum criminal negligence, worst manslaughter (read how good a lawyer you get). But if the death had nothing to do with the other person, then an autopsy would show that. And an autopsy is performed is almost all death cases to find cause. Would a dominant get suspicion is the police found a corpse tied spread-eagled and ball-gagged to the bed naked with a 10 inch dildo still on inside her, god yes. Would he be charged with murder after the autopsy found she had a congenital heart defect that caused a stroke? Yeah probably, because it contributed. But if they found same sub dead peacefully in the bed, with week old cane marks, not a chance.
