Rather than immaturity, it very well could be taken as a sign of a psychological disorder. Especially, in an environment where you are surrounded by medical professionals.

Ahem...I've worked in the medical profession for 25 years. Nobody I personally know from surgeons on down thinks twice about thumb sucking, including my own.

If the OP's partner wants to be discreet, she can do it in a bathroom stall.

How about:

A challenge to make a positive comment or smile at every member of staff or patient she comes into contact with (where appropriate).

A challenge to have all her paperwork up to date by the end of the day.

A challenge to be the one to offer to get tea of coffee for the others on her break.

A challenge to sign up to a training or improvement course to make her better at her job.

A challenge to make a list of things she wishes she was better at in her job, and work on one each week.

A commitment to supporting and helping others meet their full potential.

There are lots of things you can get her to do at work that will benefit her, her patients and her co-employees. Kinky sex game challenges? Not so much.

No. If I was the sub, I would tie YOU up for asking me to do something that could damage my job. If you have to give her something to do, make it general and positive as other posters have said.
