Generally, you should know the health history of the other person. There's more that can be transmitted than you might think. Good hygiene is important. And for the hyper paranoid, there's always the rubber dental dam so that there is no actual contact.

Funny, I have never felt squicked about doing that, but then again it is with a long term spouse, who is also very fastidious and clean as a person, and it doesn't bother me at all. If it was bothering me, I would ask them to clean and wash (using a cleaning pad then washing would be great, or vinegar and water then cleaning), but I haven't had that happen to be honest, and to me it is pretty hot to see her reaction to what I am doing:). I have heard of people doing this in d/s where it is deliberately nasty, but that I think is a bit too much...
