Shiftyw, A TENS unit should come with an electrode placement chart. The charts will show you the basic placement points to alleviate pain for a particular portion of the body. You can also google "TENS electrode placement chart". The key to alleviating pain is know where the nerve is and putting the electrode across the path of the nerve to interrupt the pain signal. You mid to upper back to your neck is not really crossing your heart path even if its on the left side of your back. Additionally your muscle tissue on you back is much thicker for most people and the heart is actually closer to your front side than your back. If you overlay an anatomy chart with a TENS electrode placement chart would see that you are not crossing the heart path with the electrode placement you described.
