Speaking as a sub male who is locked 24/7/365, if the fit is right and you have a quality belt (I said belt, not plastic piece of you know what), the one who is locked should not be able to get out, or achieve an orgasm without doing irreparable damage to himself or the belt. I am kept in a Carrera Concept belt at all times. I am allowed out for not more than one hour per month or one orgasm which ever comes first. That is the most frequent my lady will unlock me, that is not to say she is shy about simply saying that my performance was lacking this month, so maybe next month if I do better. The biggest down side to the Carrera is the rear cable is difficult to pull aside for defecation, but if you get you body on a schedule to go before your morning shower it is not a big deal. There have been times I would have done anything to achieve an orgasm, but short of destroying the belt, there is nothing you can do that wont be noticeable at a glance. Just remember, if there is no band going around the waist it is not a belt, and if it is not made of stainless steel it is not a chastity.
