If I'm aware of a personal attack or keeping up with a discussion topic where somebody has started to act combative, then I would rather confront a poster before the situation escalates or a thread ends up getting shut down as a result of acrimonious content. If I have a sneaking suspicion about an unverifiable hunch, then I should probably keep that to myself in terms of involving others, especially if it doesn't directly impact me or someone known to me. Obviously, blatant violations are outrageous enough in nature to get clamped down on (illegalities, threat of bodily harm, harassment, stalking-like behavior), and these should get reported or dealt with swiftly if it is impossible or unfeasible to get satisfactorily resolved between the parties in question, where intervention becomes necessary. However, there are those who would cry wolf when uncalled for. It's a judgment call that we each must make for ourselves, that's all.
