In that position the Domme has all the control. One strong jerk, and not in a good way, could send a man to his knees if we so desired. Men are the most vulnerable when this area is exposed and at our mercy if we so choose.
Also, a few good techniques can be put into play here as well. If a man is close to coming pulling the balls gently back down and ceasing the motion will stop it from happening. So will putting pressure on the head just below the rim.
