It doesn't have to be painful at all. It's really an exercise in trust and letting go for the recipient. Either vagina or anus will stretch to accomodate a hand. As BoiJen said in one thread, "with enough lube and patience you can back a semi into a mailbox" and that is pretty much true.
You may not be able to get the whole hand in there the first time you play at this. For me, I need to be REALLY intensely aroused to do it. It doesn't matter if I have had an orgasm before or not. Then lots of lube, lots of foreplay, lots of hot dirty talk while this is going on. My Lady also likes to use her other hand to keep my clitoris all happy while she's doing this to me. I have never torn or had problems from fisting, but we always take it slow and don't force anything.
In terms of just the physical sensation, it's kind of "meh" for me. In terms of mental, on the other hand, the fact that my Lady is caressing me so intimately, and that I have been able to give her my trust and submission to take it, that's hot.
The problems with prolapse that have been mentioned are probably not due to fisting. Your uterus and vagina are held in place internally by some very strong ligaments. Childbirth, abdominal surgeries, obesity, hormone deficiency, smoking, and probably a bunch of other things can weaken or damage those ligaments, then prolapses can happen. Having regular annual OB/GYN checkups is A Good Thing as they will tend to detect this sort of problem. And of course, if you have bits of organ turned inside out and protruding places they don't belong, CALL YOUR DOCTOR!
