For me it depends on the person i am with what term is used to describe me. When i am un-owned or in a relationship where i am allowed to choose my description, i use the word slave. The reason for using it is my favorite quote:

i use it to remind me.

I identify as *me*, overall - but I'll roll with the question ;)

I'm *his* slave, because we have agreed that he owns me.  That's all - that's the only dividing line.  Our relationship looks exactly like it did before we decided that he owned me, before I was his slave.

I'm not "a slave", or "a submissive", because not only don't I identify as such, I don't seek it out in relationships either.  My d/s is based on the specific chemistry between me and someone else.  Most often, and most preferred, it's dominant or neutral - rarely, it's submissive.

I identify as MasterK's slave because outside of our common hard limits, and anything illegal, I would do whatever was asked/ordered of me. He would not have a submissive, only a slave, and over the months of getting to know him, and negotiating with him, my desire to belong to him overcame any reservations about being his.
