Off and on since the forming of Pakistan. Every time the government tries to control the tribal regions it flares up again which is why the government most of the time lets the tribal regions more or less rule themselves..
When Europeans arrived in India, muslims were boss, when Britain planned to leave, the majority hindus would be boss. So when Britain withdrew, muslims wanted partition and a muslim country of their own and there was a big transfer in populations, meaning that many Pakistanis (and Indians) were basically foreigners in their own country. The incoming new muslims and the muslims already resident in the region that was Pakistan had different ideas of how they wanted to live. Still no agreement and they've been unable to stabilize the country which is why its had several military coups in its history. Of course, corruption by the political classes doesn't help the situation and I bet they are getting even fatter with the money the US is pumping into the country.
