Aneirin, that could be one factor and I think another is perhaps that society knows that obesity can be unhealthy due to the exertion involved in the body having to work harder at keeping those vital organs working - which is why it is a rarity to encounter an obese person that actually does have a slow metabolism. I also think another reason why it appears unnattractive to so many is that often we see larger people squeezing themselves into tighter garments in the hope it'll make them appear thinner - which, it doesn't, it just highlights the fact that they are too large for their clothes.
Even though I am a larger lady, I do have a very good self image and have no issue with appearing naked infront of others. I used to be a very trim/fit lady, I went to the gym every day and lifted weights/ran at 5am in the mornings along Wimbledon Common and STILL had a dress size of 16 even though I'm 5'3. Since changing careers (leaving a very active role for a sedentary one) the weight piled on b/c I didn't alter my intake to take that into consideration. At a dress size of 24, I recognise that I have a higher risk of serious health issues caused by the increase in weight, however, I am extremely healthy. My G.P. checks my cholesterol and glucose levels periodically and both are at the 5.0 to 5.1 level, which are acceptable, with a regular BP of 108/57.
When I dress tastefully, ie not trying to squeeze into something too small for me, I have the appearance of an attractive woman and fully understand why I would not be attractive to some people when I'm naked, which is a state I'm very comfortable in. My boy is very athletic (semi pro football) and has the body of an adonis (my preference in men), and regardless of whether I am clothed or otherwise, he is ever in lust - and I think it is all to do with my confidence, and sexual awareness.

Maybe people think, well, unclothed or in beach attire other people would be repulsed at the sight of them, so they don't bother with looking after themselves, or don't be seen in a clothing state that would invite the obvious scorn society seems to have for larger people.
Me, though not large, I used to have body image issues, but not anymore, my own place, when it's warm in here, clothing is optional, when it's not warm, no chance, I do not get on with being bodily cold. Swimwear in our climate ? No chance, I am not going to be brave and shed clothes just to freeze. Clothing the wearing of or lack of it to me is to do with air temperature, not modesty.
Hey, even through our summers when it has been warm, I have been known to wear warm clothing, a sweater even, but on the hottest days when people are melting, I can walk around normally and without bother, wondering why people are moaning so much at the rare good weather.
