While I'm not saying it was definitely planned and that the pregnant lady was planted their as a prop, I'm also not naive enough to think that a situation came up that was used for political gain. She was swaying before anything happened, as Joether pointed out. The young lady did help as the pregnant woman was swaying, as you'd expect. Obama seemed to turn around just a bit early to be a reaction to her faint. He puts a hand on her shoulder, then turns back to the microphone to make a joke (this is what happens when I talk too much [paraphrased]) and then turns back to the woman. Once Obama turned to the pregnant woman (her legs buckle), the lady that had her arm around her made no motion to help. That's a bit conspicuous. It's entirely possible the event went as reported by the media. It's also possible the event was staged. I don't believe either is completely accurate. I do believe the action wasn't pre-planned, but was used as a tool by the President. I'm also not so naive to think that President Obama is the first President to use such tactics. I have not seen evidence of any other President doing so. Nor, have I heard any claims that happened. I take anything with a skeptical view to start anymore (since 2005/6).
