I'm 4'8.5".  I LOVE heels.  What makes heels comfortable for me is having wider chunky heels, and no toes.  The higher the heel the more force is put on the ball of the foot and the toes.  Not having my toes crammed against the wall of the shoe works well for me.  Round toes like Mary Janes are a close second.   Strappy sandals are wonderful.  I guess as long as theres no more than say 4 inches  rise, its comfy.  Platforms are a godsend.  I have 7 inch heels with  2.5 inch platforms.  They are pretty comfy actually.  My MJ's are 5 inch heels with about a half inch platform, and I can run in them.
As menetioned before, having cushioning in the ball of the foot area can really make all the difference, regardless of heel height.
I  work in a lab so  its usually  running shoes for me for work.  Although, I do on occasion get to wear my MJ's in the winter.

To My day job I wear 2 to 4 inch heels, depending on the style.  For play I wear 5 or 6, depending on the scene.  For personal time with My personal slave ... tennis shoes *lol*.

Any woman that wears 4 inch heels for work and 6 inch heels for play is my kinda woman!

Yeah, that's true, but as Susan mentioned, those gel insoles are wonderful.  They make a huge difference for me.  I love the way I look in heels so I like them to be comfortable.

2-3 inches at work

4-5 if I know I only have to wear them for a short period of time

I have a pair of 6 inch boots, but after 5 hours of wearing them at my last event, my little toes were numb for 2 days.

Barefoot with a perfect pedicure is my personal choice.

Okay, now THIS is a subject i know.

The shape and angle of the shoe makes all the difference. Whatever the angle is that they use for most shoes below 3", it does NOT work for me, so i don't even bother. It's 3" plus. I also go totally form over function, so i love pointy toes, spiky heels, some stacked chunkier ones (as long as they don't look chunky), and will not wear wedgies and am very picky about platforms (besides, i have felt my ankles wobble in those and they can feel so heavy that i can't walk). when i was an undergrad, I even played impromptu games soccer in my heels and i don't know how to dance in anything but. The streets are problematic for high heels in the UK i have found so i have been having great diffuclty adapting.

Besides heels, it's barefoot barefoot!!! or totally flat flip flops. Yes somehow, must find a compromise. My current pair of favourites are totallly comfy strappy fuscia 5" shoes with tacky gold heels, LOVE EM! (the key is that they have padding beneath the ball of the foot -- why don't ALL shoes do that, hmmm?). So for me, favorite heights are 3-5". Only wore 6" CFMP's once (and i wasn't on my feet).

Mind you, i have had numerous foot surgeries to deal with problems, but they were worth it! Having said that, since my Master collared me i have been much more careful and am conscious of not allowing damage to occur to his property, (smiling happily).

 It is the shape of the shoe and the angle of the foot that makes all the difference.
Plus the material on the sole needs to be flexible and cushioned. 
 
When you look at a sexy shoe, consider the angle of the foot inside the shoe, and imagine walking on tippy-toe with all your weight on the ball of your foot while your toes are cramped. 
 
I would also like to suggest, (Popeye) that you find a pair of spikes to fit your foot and try walking around in them and see how you like it.  I intend to find a pair for my slave, who is a foot and shoe freak.  This way he will know both sides of the coin! LMAO.  (I do hope he reads this)  
 
I personally will not torture my feet for fashion.  If it hurts, I won't wear it for long.

I just love it when I where my wooden 6 inchers that have NO heel.  I get compliments all the time and asked how to where them.  Practice, practice, practice.  Basically all you are doing is balancing on the balls of the feet all the time.  You can not walk heel first in them.  Many women have tried them on and just fall flat.  And they are so cool to look at too.
 
The day we bought them we were in Evansville, IL at a shop that sold clothing, shoes, etc. mainly for exotic dancers.  I was trying on shoes left and right and of course modeling them for hubby.  I actually had some of the customers who were dancers compliment me on my ability to wear and walk right in them.  I think they were shocked to know that there actually were women out there that were them for fun.

2 inch heels for me, I have a bad back. When I'm scening I sometimes wear a 3 inch, no higher then that though as I can't walk in them and I find them totally uncomfortable. I'll take a nice tennis shoe or combat boots anyday
