I think fear is very real. I had a friend once who suggested taking girls out on a date/picnik on a swinging rope bridge over a small creek. He said they would whine and complain for a while and he would have them put the sandwiges together or whatever, then after a short time he would put his hand down their pants and they would be dripping wet.

His theory is that girls get horny from fear.Go figure.

Daddy and I do a lot of knife play, it's something I really enjoy. He likes it as well but he's not into "blood play" so he rarely if ever actually cuts me and if he does they're surface cuts with minimal bleeding.
It's still "real" even without the cutting, it still gets me into that headspace and I never really know if Daddy will cut me or not until after everything is done. I've even gotten the giggles during knife play but that doesn't mean it wasn't a good scene.

It's because they are dumb and their brains don't work right. Girls get scared and their panties get wet.

Sometimes to avoid unnecessary and boring foreplay I just tell them that they are pregnant and head straight for the fisting.

i like the feel of the cold steel on my flesh after a good flogging to warm the skin up. i have also been cut to place my Mistress initials on my chest as both of us enjoy the bonding. i know that my Mistress can and will cut me if She wants to so it is very real to me as it has happened it is not a hard limit.
