I've certainly seen it happen, in central California (not North Carolina). I forgot to mention the most egregious example of them all. How many American men have gotten married and adopted their wives' last names? Not hyphenated, not hybrid, just a straight up trade of his last name for hers. Talk about a taboo that would make family and friends suck the air out of the room with their collective gasping. I don't think I'd even have the nerve to try such a gambit.
