Not to get bogged down in this, but the postmortem on that trainwreck is worthy of a thread all of its own, although I don't think such a thread will be possible without either diligent self-moderation or external moderation, simply because of the level of emotion involved in what happened for a lot of the people involved. Bob started the causal chain, and it's fair to put that on him. We all had a million points along the way where we could have averted what eventually happened, however, without ever taking the opportunity to do so.
