If you're talking about tearing or popping a mucus membrane or something... well, yeah, that's also a risk in anal sex... and in vaginal fisting... and insertion of anything anywhere in your body. It doesn't really have the capacity to cause worse irreversable damage than vaginal fisting, just different. :\ In either case you need to keep your head about you (and your partner does too, especially).
